We are working on a very special video project and need your help.  We are collecting videos of vapers and compiling their stories in an amazing way.  Want to help?

Here’s how:

Bust out your favorite selfie stick and make a short video of yourself answering the following:

  • Name
  • Age
  • How long you smoked
  • How long you have been vaping
  • What if anything vaping has done to improve your life
  • What flavor(s) of eliquid helped you make the switch
    • No brand names please, give the flavor profile

In consideration of my participation in the video production, I hereby consent to the photographing, recording, or reproduction in any other manner of my likeness, voice and activities, in whole or in part, including the use of video and audio recordings and web use (hereafter “recordings”). I hereby waive any right that I may have to inspect or approve the finished audio and/or video recordings that may be used in connection herein. I further grant Rising From The Ashes Podcast and their agents or assigns, exclusively and perpetually throughout the world and elsewhere, all rights of every kind, whether known or unknown, for unlimited use, reproduction, distribution and presentation of such recordings now and in the future. I understand that I will receive no monetary compensation for my participation in this production, nor may I make any monetary claim in the future for this participation. I hereby release and hold harmless Rising From The Ashes Podcast and their officers, employees, agents, or assigns from any claims.

I hereby assign, transfer, or otherwise convey to Rising From The Ashes Podcast, and their agents or assigns all rights, titles, and interests in and to the video(s) submitted, including without limitation all copyrights and other intellectual property rights therein. Furthermore, I agree that any and all of the submitted video(s) may be used, edited, reproduced, distributed and presented in any way deemed proper by Rising From The Ashes Podcast, its agents or assigns without additional consent, or payment to me or anyone else claiming to have a right to such payment. There is no time-limit on the validity of this release nor is there any geographic limitation on where these materials may be used, distributed or presented.

Video submissions may be used in any way, including but not limited to the following:

•Presentation in a public theater(s)
•Educational presentations
•Integration into social marketing campaigns and programs (e.g. posted on websites, played in movie theatres, shared through YouTube and other social media channels and broadcast on TV).

I represent and warrant that the video created for submission to the Rising From The Ashes Podcast is original and does not unlawfully infringe upon the copyright, trademark, patent or other intellectual property rights of any third party, or is in the public domain. If the video(s) created for submission to the Rising From The Ashes Podcast becomes the subject of a claim, suit or allegation of copyright, trademark or patent infringement, Rising From The Ashes Podcast shall have the right, in its sole discretion, to reject the submission. I agree to indemnify and hold harmless Rising From The Ashes Podcast, its officers, employees, agents or assigns from and against any and all claims, actions, costs, judgments or damages of any type relating to the production or distribution of the video(s) submitted to the Rising From The Ashes Podcast.
